A pregnant Beth visits her sister Ellie in a condemned apartment complex where an earthquake uncovers a hidden chamber containing a demonic book. Ellie becomes possessed and attacks her family and neighbors, with Beth and her niece and nephew fighting to survive. They discover that the only way to stop the demonic possession is to destroy the host. Ellie's possession merges with her children's to create a monstrous entity, but Beth and her niece manage to defeat it and escape.
