How Surgeons Fix a Broken Tibia Near the Ankle Without a Plate

Distal tibial fractures sit in one of the most unforgiving zones in orthopedic trauma. The bone just above the ankle has a thin soft tissue envelope and limited blood supply, so traditional plating through larger incisions can raise the risk of wound complications. That is why I want to highlight a newer minimally invasive option: the retrograde distal tibial nail.

Here is how it works in the operating room. Instead of opening the fracture site, we make a small incision on the inside of the ankle at the medial malleolus. Under live fluoroscopic guidance, a short, anatomically curved nail is passed upward through the marrow canal of the tibia, crossing and stabilizing the fracture from the inside. Locking screws are then placed through small stab incisions, anchoring the nail so the fracture cannot rotate or shift. Because the nail sits within the bone, it shares load naturally and preserves the outer blood supply the bone needs to heal.

The benefits are compelling: smaller incisions, less soft tissue disruption, a stable construct that often allows earlier protected weight bearing, and no violation of the knee joint, a common source of pain after traditional tibial nailing from above.

Like any procedure, there are risks. These include screw malposition, irritation or injury to tendons near the ankle, fracture of the medial malleolus during insertion, loss of reduction, and infection. Careful technique and fluoroscopic confirmation at every step help minimize each one.
