The Hidden Cause of Young Athletes' Hip Pain

As an orthopaedic surgeon, one of the most overlooked causes of hip and groin pain I see is femoroacetabular impingement, or FAI. It develops when the bones of the hip joint form with a slightly irregular shape, so the ball and socket no longer glide smoothly. With repeated motion, that abnormal contact wears on the labrum and the articular cartilage that cushions the joint.

FAI shows up in three patterns. Cam impingement involves extra bone on the femoral head and neck, most common in younger, active patients. Pincer impingement involves overcoverage at the rim of the socket. Many patients have a combination of both.

Symptoms often start quietly. A deep ache in the groin, stiffness, or a pinch when squatting, sitting for long stretches, or rotating the hip. Athletes tend to notice it earlier because they load the joint harder, but activity itself does not cause FAI.

When a patient comes to me with these complaints, I start with a focused exam and imaging. X-rays reveal the bony shape, while MRI shows damage to cartilage and the labrum. A diagnostic injection can confirm the hip as the true pain source.

Most patients improve without surgery through activity changes, anti-inflammatory medication, and targeted physical therapy to strengthen the muscles that support the joint. When symptoms persist or imaging shows real joint damage, surgery can reshape the bone and repair the labrum, often arthroscopically through small incisions. Left untreated for years, FAI can advance toward hip arthritis, which is where joint replacement enters the conversation.
