Radiological Signs in Esophageal Disorders
In this high-yield radiology lecture, we review the important radiological signs associated with esophageal disorders, a favorite topic for USMLE Step 2 CK and an essential part of clinical gastroenterology. Imaging, particularly the barium swallow study, provides classic visual patterns that help diagnose structural and motility disorders of the esophagus.
We begin with achalasia, which shows the classic โbirdโs beak signโ due to smooth tapering at the lower esophageal sphincter from impaired relaxation. In esophageal carcinoma, barium studies may reveal an โapple-coreโ or โrat-tailโ lesion, reflecting irregular, constricting masses. Diffuse esophageal spasm produces a โcorkscrew esophagusโ appearance, caused by uncoordinated contractions. Esophageal varices, on contrast studies, demonstrate serpiginous filling defects, while Zenkerโs diverticulum appears as a posterior outpouching just above the upper esophageal sphincter.
Each radiological sign is linked to its clinical presentation such as dysphagia, regurgitation, chest pain, or risk of aspiration, making these findings not only diagnostic but also clinically significant. Radiological signs are also correlated with advanced diagnostic tools such as endoscopy and manometry for comprehensive understanding.
By combining radiological patterns, underlying pathology, and clinical significance, this lecture equips medical students, residents, and healthcare professionals with exam-ready knowledge for Step 2 CK case-based questions and practical interpretation skills in Western medical practice.
