Imaging, particularly the barium swallow study, provides classic visual patterns that help diagnose structural and motility disorders of the esophagus. We begin with achalasia, which shows the classic โ€œbirdโ€™s beak signโ€ due to smooth tapering at the lower esophageal sphincter from impaired relaxation. In esophageal carcinoma, barium studies may reveal an โ€œapple-coreโ€ or โ€œrat-tailโ€ lesion, reflecting irregular, constricting masses. Diffuse esophageal spasm produces a โ€œcorkscrew esophagusโ€ appearance, caused by uncoordinated contractions. Esophageal varices, on contrast studies, demonstrate serpiginous filling defects, while Zenkerโ€™s diverticulum appears as a posterior outpouching just above the upper esophageal sphincter. Each radiological sign is linked to its clinical presentation such as dysphagia, regurgitation, chest pain, or risk of aspiration, making these findings not only diagnostic but also clinically significant. Radiological signs are also correlated with advanced diagnostic tools such as endoscopy and manometry for comprehensive understanding.
