Case Report: An unexpected cause of right upper quadrant abdominal pain in a teenager

Abstract:
A previously healthy, sexually active 17-year-old female was admitted to our hospital with a 4-day history of right upper quadrant abdominal pain associated with high fever, vomiting and diarrhea. On examination there was tenderness in right hypochondrium. Gynecological exam revealed sparse vaginal bleeding and pain on deep palpation and mobilization of the cervix and adnexa. Abdominal computed tomography scan showed signs of inflammation of the pelvic adipose tissue.

A 2-month-21-day old male infant presented with respiratory distress, jaundice for 4 days, intermittent clay-coloured stools for 12 days and one episode of convulsion. He was diagnosed with moderate ventricular septal defect via echocardiography on 25th day of life and was on furosemide and digoxin. He was born at term via vaginal delivery with birth weight of 2.4 kgs. An elder sibling died at 3 months of age due to congenital heart disease with neonatal hepatitis.
Is this congenital (cCMV) or acquired CMV (aCMV)? How to treat it?

A 6-year-old caucasian boy presented to a Pediatric appointment with a longitudinal and linear brown pigmentation in his right hallux fingernail. The nail alteration extends from eponiquium to the free edge of the nail plate. This asymptomatic alteration appeared about ten months before and had not changed in thickness, texture or pigment intensity. There were no other alterations on physical examination including in the remaining nails.
What is the most likely diagnosis?

Wilson’s disease (WD) is a hepatolenticular degeneration caused by mutations in the ATP7B gene. It is an autosomal recessive disorder resulting in excessive copper deposition, particularly in liver, eyes and brain. This occurs due to a deficiency of ceruloplasmin, a copper binding protein, often leading to liver cirrhosis. A characteristic ocular sign of Wilson disease is the Kayser-Fleischer (KF) ring, which represent copper deposition in Descemet’s membrane of the cornea.

Case Report: Protein-Losing Enteropathy as a Rare Complication of Giardiasis in Children

Abstract:
Giardiasis, caused by Giardia lamblia, is an intestinal infection that presents various clinical manifestations. While malabsorption is a recognized complication, protein-losing enteropathy (PLE) is infrequent. This report describes a 17-month-old healthy girl who presented to the emergency department exhibiting generalized edema attributed to hypoalbuminemia, which resulted from gastrointestinal protein loss. Subsequent investigations
identified giardiasis as the underlying etiology.

A 24-day-old female newborn presented to the pediaatric emergency department due to erythematous-violaceous cutaneous lesions in both inferior limbs that did not resolve with local warming. The patient was born full-term at 40 weeks and 4 days, with an APGAR score of 10 at both 1st and 5th minutes and a birth weight of 3145 grams. The patient’s parents were unrelated and there were no significant prenatal ou perinatal complications.

Pediatric vasculitis entails a heterogeneous group of disorders characterized by inflammation of blood vessels that can affect multiple organs. IgA vasculitis is the most common form in children, classically presenting with palpable purpura, arthralgia and potential gastrointestinal tract and renal involvement. While IgA vasculitis is an established diagnosed based on characteristic clinical features, it remains important to consider other etiologies.

In resource-limited countries, when neonatal bacterial infection is suspected, empirical antibiotic therapy is initiated with a triple combination of a third-generation cephalosporin (Cefotaxime or ceftriaxone), ampicillin or cloxacillin and Gentamycin. While this approach has been effective in past decades, it is now showing its limitations. With the emergence of resistant bacterial strains, this probabilistic protocol is experiencing increasing failures in the management of neonatal infections.

A 3-year-old child arrived at the emergency department with drowsiness and generalized hypertonia, raising suspicion of encephalitis.

Initial imaging, lumbar puncture, and hospital toxicology screening were normal.

However, further forensic toxicology analysis later confirmed exposure to cocaine, trazodone, and aripiprazole.

📌 This case highlights the importance of considering toxic exposure in pediatric neurological symptoms, even when early screening tests are negative.
