A 21-year-old female university student presented to a primary healthcare clinic concerned about possible HIV exposure after unprotected sexual intercourse approximately 18 days earlier.

She had no significant past medical history, was not taking chronic medication, and reported being generally healthy.

Initially, she felt completely normal and did not think much about the encounter. However, after reading online about early HIV symptoms and learning that they can mimic a common flu, she became increasingly anxious.

When she later developed symptoms that felt like a mild viral illness, she began to worry that this could be more than just a seasonal infection.

She denied genital ulcers, abnormal vaginal discharge, or pelvic symptoms, but remained fearful that she may have contracted HIV.

Her question was simple:

A 75-year-old woman presented to the surgical outpatient department with a 4-month history of intermittent abdominal discomfort and progressive abdominal distension. She reported vague, dull pain localized mainly to the lower right side of the abdomen, associated with early satiety and reduced appetite. Over the preceding 2 months, she had unintentionally lost approximately 6 kg. She also complained of occasional nausea, increasing fatigue, and altered bowel habits characterized by intermittent constipation.

On examination, the patient appeared mildly cachectic but remained haemodynamically stable. Abdominal examination demonstrated mild abdominal distension with tenderness localized to the right lower quadrant. A firm palpable mass was appreciated in the right iliac fossa. There were no signs of peritoneal irritation, as rebound tenderness and guarding were absent. Bowel sounds were present and normal on auscultation.

Abdominal ultrasound demonstrated a complex cystic lesion in the right lower abdomen.
Contrast-enhanced CT abdomen and pelvis showed a dilated appendix with irregular wall thickening and surrounding soft tissue changes, raising suspicion for appendiceal neoplasm.

Hypothyroidism occurs when the thyroid gland produces too little thyroid hormone, causing the body’s metabolism to slow down. This can make a person feel tired, sluggish, cold, constipated and gain excess weight.

A 55-year-old woman visited her doctor with tiredness, shortness of breath, dizziness, weakness, heart palpitations, and numbness in her feet that had been getting worse over 3 months. She had high blood pressure, followed a mainly vegetarian diet, and had no history of bleeding or recent illness.

Her blood tests showed severe anaemia and a very high LDH level, suggesting increased breakdown of blood cells. Further tests found very low Vitamin B12 levels, while other vitamin and iron levels were normal.

A diagnosis of Vitamin B12 deficiency and anaemia was suggested.
