Selective Thoracic Fusion in Adolescent Idiopathic Scoliosis – Precision Over Aggression

This case highlights a young girl with a Lenke 1 curve with a C modifier, where the key surgical objective was not just correction, but achieving optimal global balance.

A selective thoracic fusion was performed, deliberately preserving the lumbar mobility. Instead of maximal correction of the thoracic curve, a strategic hypocorrection was carried out. This allowed the lumbar curve to spontaneously correct and align, maintaining coronal balance without extending fusion levels unnecessarily.

Equally important was the sagittal plane restoration. The preoperative dorsal hypokyphosis was addressed using a cantilever mechanism, enabling effective kyphosis correction and restoring physiological thoracic contour.

The postoperative images demonstrate:
• Excellent coronal deformity correction
• Well-balanced lumbar spine without direct intervention
• Restoration of dorsal kyphosis
• Stable construct with optimal alignment

This case reinforces an important principle:
In scoliosis surgery, success lies not in maximal correction, but in harmonious balance.
