After decades of clinical practice, teaching, research, and continuous refinement, I am pleased to announce the first published release of the 4R Operational Diagnosis (4ROD) Protocol.

This publication represents much more than two articles.
It introduces a comprehensive, structured diagnostic framework to guide clinicians in collecting, interpreting, and integrating diagnostic information before treatment begins.

The protocol consists of four sequential components:
• R1 – Patient Report & Rapport: transforming the patient's story into organized clinical data through structured communication and examination.
• R2 – Radiographic Findings: introducing the 10 Areas of Diagnostic Interest (10 ADI), a systematic approach to interpreting periapical radiographs.
Future publications will continue with:
• R3 – Response Testing
• R4 – Restorative & Structural Assessment

My sincere hope is that this protocol will stimulate discussion, encourage critical thinking, and contribute to more comprehensive, biologically based diagnoses in endodontics.

The PPPQ was developed during AbouRass's advanced studies at the University of Pittsburgh and has been refined over five decades. The PPPQ transforms the patient pain story assessment into a structured, patient-centered diagnostic tool.
