What is a Fail-Safe Position?

A fail-safe position is the position a control valve automatically moves to when the instrument air supply or power is lost. This position is selected during the design stage to keep the process in the safest possible condition.

What does Fail Open (FO) mean?

A Fail Open (FO) valve is designed to move to its fully open position whenever the instrument air supply fails.

In this question, when the air supply is completely lost, the actuator spring drives the valve to its fail-safe position, which is 100% open.

Key Point:
Fail Open describes what the valve does after an air failure. It does not describe the valve's normal operating position.

Why?

A standard pressure transmitter calibrated from 0–10 bar has an output of 4–20 mA, not 0–20 mA.

✅ When the transmitter input is 0 bar, the output is 4 mA (Live Zero).
✅ When the transmitter input is 10 bar, the output is 20 mA.
✅ An input of 5 bar represents 50% of the calibrated range.

Therefore,

Output = 4 mA + (50% × 16 mA)
= 4 mA + 8 mA
= ✅ 12 mA
So,


⚠️ Common Mistake:
Many people selected 10 mA because they assumed the transmitter outputs 0–20 mA. In reality, industrial transmitters use a 4–20 mA live zero, so the correct answer is 12 mA.
