Quick QUESTION: during my last shift I saw a 41-year-old patient with no significant medical history who came into the ER with right upper quadrant abdominal pain, nausea, vomiting at home, and fever episodes, symptoms that started about 2 days ago.
On physical exam, she had a positive Murphy sign. Lab results showed leukocytosis around 17K and a CRP of 50 mg/L, no anemia, normal lipase, and no signs of cholestasis, hepatocytolysis, or other abnormalities.
What imaging would you order first, and what diagnosis are you thinking?

A 45-year-old woman presents with 6 months of symmetrical joint pain in the hands and morning stiffness. She also reports progressive fatigue and mild unintentional weight loss over the past several months.
On examination, she has a malar rash sparing the nasolabial folds, which is photosensitive. We can also see mild swelling of several small joints.
Laboratory tests show anemia, leukopenia, thrombocytopenia, and elevated ESR and CRP. Urinalysis is normal.

Hi everyone, I was thinking about creating short USMLE-style quizzes from time to time, so you can test your medical knowledge!
A 32-year-old man comes to the clinic because of recurrent, painful mouth ulcers for the past 18 months. He also reports episodic genital ulcers and recent redness and pain in both eyes. On physical examination, in addition to shallow aphthous ulcers on the buccal mucosa, he has tender nodular lesions on the lower extremities and his eyes appear injected with findings suggestive of anterior uveitis on slit-lamp evaluation.
What do you think? Based on the clinical picture, what’s the most likely diagnosis?
Drop your thoughts in the comments!
