These words hit like a punch to the gut: We’ve tried everything, We’ve run out of options. For people living with Crohn’s disease who have cycled through medication after medication with no lasting relief, this moment is both terrifying and isolating. But here’s what both patients and doctors need to know, running out of standard treatment options doesn’t mean running out of hope.

What Happens When Medications Stop Working?
A significant number of people with moderate to severe Crohn’s disease either don’t respond to treatment or lose response over time, even after trying multiple biologics.

These individuals often experience:
Ongoing inflammation despite aggressive treatment
Fatigue, pain, and poor quality of life
Higher risk of complications like strictures, fistulas, or abscesses
The emotional toll of feeling like their body is unfixable
Life-altering decisions about surgery or long-term disability

For Healthcare Providers: When the Playbook Runs Out
When the traditional treatment algorithm fails, it’s time to rethink the strategy. Here’s what providers can do:

Back to the Basics: Reassess & Realign
Reevaluate medication adherence, therapeutic levels, and antibody development

Many doctors are unaware that Inflammatory Bowel Disease (IBD) presents differently in Black patients, which can lead to misdiagnosis, delayed treatment, and increased suffering.

The lack of awareness and cultural competency in medical settings often means that symptoms are dismissed or attributed to other conditions.

Here are three critical steps to take if you suspect you have Inflammatory Bowel Diease (IBD):

1. Track your symptoms carefully: Document pain, fatigue, skin conditions, and joint pain. A detailed symptom history can help ensure your concerns are taken seriously.
